All appointments are private pay and are 53 minutes in length.

• Initial Assessment: $200

• Follow-Up Appointment: $150

• 30-Minute Follow-Up: $75 (available by request only)

I accept FSA and HSA cards as payment. Payment is due at the time of service.

At this time, I do not offer sliding-scale rates or payment plans.

Using Out-of-Network Insurance Benefits

While I do not bill insurance directly, I can provide a superbill for you to submit to your insurance company for possible out-of-network reimbursement.

Because insurance coverage varies by plan, it is your responsibility to contact your insurance provider to confirm your out-of-network nutrition counseling benefits and reimbursement rates before your appointment.

Please note that superbills require a diagnosis code. We will discuss this during your appointment and determine the appropriate diagnosis code based on your individual circumstances.
